1. 活動名稱：第五屆台灣國際方塊舞大會
2. 主辦單位：中華民國方塊舞協會
3. 活動日期：2011年4月 9日 (週六) 14:00～22:00及
                        2011年4月10日 (週日) 10:00～16:00
   ※會前會"Tony's night" 4月8日 週五 19:00～21:30
4. 活動地點：交通部國際會議中心
                    台北市中正區杭州南路一段24號
5. 活動內容：方塊舞（分為Mainstream, Plus, Advanced及C1等級）、
             圓舞（Round Dance）、排舞（Country Western/Line Dance）等，
             由中外Caller主持，並分為二個廳同時舉行。
6. 報名費用：（包括活動期間之活動及保險費用）
(1) 2010年12月31日之前
※中華民國方塊舞協會會員 NT$1,500元(含餐費 ~ 第一天西式Buffet晚餐+
   第二天便當午餐)， 或每人NT$1,300元(不含餐費)
※ 非會員以上費用加收300元。※ 國外參加者 US$60.00 (含餐費)
(2) 2010年12月31日之後
※中華民國方塊舞協會會員 NT$1,700元(含餐費 ~ 第一天西式Buffet晚餐+
   第二天便當午餐)，或每人NT$1,500元(不含餐費)
※ 非會員以上費用加收300元。※ 國外參加者 US$65.00 (含餐費)
7. 餐點：第一天(週六)晚餐為西式Buffet(憑大會名牌識別顏色就位).
                第二天(週日)中午為便當(由各社團派員領取).

     匯款入專戶，亦可直接寄交上述大會籌備委員會祕書處辦理報名。

9. 取消辦法：
(1) 2011年2月28日前以書面通知大會籌備委員會祕書處取消報名或訂餐者，
     扣除10%手續費後退費。(以郵戳為憑)
(2) 2010年3月1日之後取消者，一概不予退費。
10. 注意事項：
(1) 活動期間，參加者請務必穿著方塊舞服裝。
(2) 參加者須憑『報名確認單』辦理報到，並領取大會名牌。
(3) 活動期間(含4月8日會前會)，參加者須憑大會名牌入場，並須全程配戴，不得私自轉讓。
(4) 參加餐點者，憑大會名牌識別顏色用餐。
(5) 若原定之活動場地因故無法使用，承辦單位有權更改活動地點。
11. 本活動辦法如有未盡事宜，得隨時修正不另通知。

Square Dance (Mainstream, Plus, Advanced, C1); Round & Line Dance
hosted by Chinese and Foreign callers and simultaneously held in two ballrooms.

6. Registration Fees (activity and insurance dues included):

forwarded/remitted by square dance community leaders to the Secretariat.

● After the registration form and fee are received and approved by the Convention, a “registration confirmation” will be issued to the applicant to complete the registration procedure.

9. Cancellation:
(1) Prior to February 28, 2011(based on postmark date)
-Refund with charge of 10% handling fee (to cancel registration or meal must send a written notice to Secretariat of the Preparatory Committee).
(2) After February 28, 2011
-No refund for cancellation.

10. Reminders:
(1) Participant be sure to wear square dance attire.
(2) The “registration confirmation” is a must to check in and to receive your convention badge.
(3) Wearing your convention badge as the “open sesame” to all convention
activities including advanced convention on April 8.
Unauthorized transfer of convention badge is not allowed.
(4) Dinner attendee will be seated according to color of convention badge.
(5) The organizer reserves the right to change the venue for reasons beyond its control.
